All templates needed to complete the Academic Program Review process are available.  

• Self-Study template, used for non-accredited programs to complete their review 

• Self-Study for Externally Accredited Programs template, used for accredited programs to complete the abbreviated program review in addition to their accreditation report 

• APR External Reviewer Report template that the external review team uses to conduct and submit their report 

• The Conflict of Interest Statement for external reviewers to complete upon agreeing to participate in the review 

• Action Plan template for the continuous improvement plans developed by the program as a result of APR 

• Mid-Cycle Progress Report template used for reporting on progress on the Action Plans after 3 years and (if required) 5 years
